Mercy Health Orthopedics and Sports Medicine will offer sports physicals for any athlete entering grades 7-12 this fall on Wednesday, May 10 from 6:00-7:00 p.m. in the Milford High School Cafeteria. The cost is $20 per athlete and a parent or guardian must accompany the student-athlete. Click here for a flyer with more information.

 

Students/parents need to bring a completed parental consent form and complete the first four pages of the OHSAA Physical Form. Both can be accessed by clicking on the respective links.

 

mCORE (Mobile Cardiac Overview and Risk Evaluation) will also be at Milford High School on Wednesday, May 10th from 12:00pm - 8:00pm.  This is an optional heart screening that is available to those who wish to participate.  The cost for this screening is $75.  Registration instructions are included here.
